Registration Forms
Returning Students
Each Fall all returning students must submit the following forms to re-register: Student Pledge, Teacher Pledge, Volunteer Commitment, Emergency Information, Medical Release Form, Information Sheet, Lunch Form, Home Language Survey, Graduation Requirements.

Fall Admissions
Applications received at CHAMPS on or prior to the first Monday in February are considered for random, public lottery. When applications are received after this date, the student is placed on a waiting list.

The lottery is held at 10:00am the first Friday in February. The list of accepted students is posted in the school office and on the CHAMPS website under the Our School menu, the Lottery Info link. Students not chosen in the lottery are placed on a waiting list in the order their application was received.

Registration packets may be picked up at the office or downloaded (at left). All required registration forms and documentation are due on or before the third Friday in February.

Waiting list
As vacancies become available, students on the waiting list will be informed by telephone of their acceptance. These students will have two weeks within which to obtain and complete the registration packet. If all the required forms and documents are not received on or before this date, registration is not considered complete, and the student will forfeit his or her priority status.
